Terms of Service
Little Poke Phlebotomy (LPP) is responsible for drawing and delivering laboratory specimens. It does not collect specimens without a physician's order or a laboratory request. LPP cannot give medical advice, treat, or diagnose anyone.
LPP needs patient information, a copy of the prescription, and a copy of the insurance card (front and back) 72 hours before the scheduled appointment. The scheduled appointment will be canceled if we do not receive the requested information in the mentioned time range. Adhering to this timeline gives us adequate time to prepare for the blood draw and ensure we perform our job correctly.
LPP handles the delivery of all samples to their proper laboratory. If a sample is being collected for a laboratory that needs to be picked up via courier service, LPP is not responsible for any delay in arrival due to weather conditions or any other problems that may occur once the sample has been delivered or picked up by courier service.
LPP is not responsible for any errors that may occur at the laboratory. Once received by LPP, specimens are the lab's responsibility to process. In the event of a lab error, LPP will gladly return and recollect the sample at a discounted rate. Any error made by LPP will be corrected at no extra charge as long as all correct paperwork was adequately submitted prior to the appointment.
Any billing questions or problems for tests collected for this service should be directed to your insurance company or the laboratory billing department.
LPP collects payment for services the day before the appointment. LPP does not bill insurance companies. The client must submit claims, and reimbursement is not guaranteed.
Please be advised that Little Poke Phlebotomy is not responsible for outstanding balances with LabCorp or Quest. These balances must be settled before a scheduled home lab draw. The labs have the right to decline specimens of patients with outstanding balances.
LPP is an independent convenience service not affiliated with laboratories or doctor’s offices. It is not authorized to give discounts for lab tests collected.
Under no circumstances will LPP give out clients’ personal information to unauthorized personnel. Client information is always kept confidential. LPP does not receive any results. If your doctor does not receive results, it is the responsibility of the doctor's office to contact the lab. Most of the time, they have an account with the labs.
Please be considerate. If an appointment needs to be canceled, LPP needs 48 hours' notice. We send confirmation out of courtesy. If you are a no-show, you will be charged an additional $25 to reschedule. We make it easy for you to contact us with any questions or concerns. We may not always be available by phone, but you can also reach us via email or text.
